The changes will be challenging given the vagueness in payer feedback regarding History and Exam not counting toward the overall level of service, which means Medical Decision-Making and/or Time in conjunction with “Medical Necessity” will drive your services. This will put pressure on provider documentation since it will rely more on clinical judgement than anything else.
